Definition

Learning from Incidents (LFI) is the organisational process of turning incidents, accidents, near misses and other failures of control into changes that reduce the risk of recurrence. It extends beyond completing an investigation. Effective LFI includes reporting events, deciding what requires investigation, analysing causes and failed barriers, planning and implementing actions, sharing relevant learning and checking whether the resulting changes are effective. It also uses patterns across multiple events to identify systemic weaknesses.

Why it matters

Completing an investigation does not by itself show that an organisation has learned. Quality, HSE and Operations managers need to know whether lessons reached the people and processes at risk, whether recommendations resulted in real changes and whether similar events stopped recurring. That requires looking across incidents for repeated causes, tracking actions through implementation, verifying effectiveness and updating procedures, risk controls, training or management arrangements where necessary. Learning from Incidents turns individual events into evidence for continuous improvement and helps prevent the same failure from appearing elsewhere.
