Definition
A safety observation is a recorded observation of a workplace condition, behavior or practice that is relevant to occupational health and safety. It can identify an unsafe condition, a weak control, a positive practice or another opportunity for improvement without requiring an injury, incident or near miss to have occurred. Safety observations are not a universally standardized event category, so organizations should define how observations differ from incidents, near misses and formal hazard reports.
Why it matters
For HSE and operational teams, observations provide a proactive source of information before harm occurs. Consistent recording allows teams to assess potential impact, identify recurring hazards or control weaknesses and decide whether follow-up, corrective action or broader risk assessment is needed, while also recognizing practices that should be reinforced.
Describe the condition, behavior or practice factually, including location and relevant context without assuming a cause before assessment.
Record what was observed
1
Consider what could reasonably happen if the condition persists or recurs, including likelihood and possible consequence where useful.
Assess potential impact
2
Identify barriers, procedures or practices that are working, missing or weaker than expected.
Consider existing controls
3
Choose whether the observation needs immediate correction, preventive action, risk assessment, investigation or simply documented learning.
Decide proportionate follow-up
4
Review repeated observations across locations or activities to detect recurring hazards and reinforce effective safe practices.
Use observations for learning and trends

Turning a safety observation into useful follow-up
The value of an observation comes from capturing enough context to understand potential risk and decide proportionate follow-up, not from turning every observation into a formal incident investigation.
An unplanned event or condition that resulted in, or could contribute to, harm, loss, nonconformance or operational disruption.
Incident
Event
· Incident Management
An event that did not result in injury, damage or loss but had the potential to do so under slightly different circumstances.
Near Miss
Event
· Incident Management
The structured process for reporting, assessing, investigating, following up and learning from incidents and related events.
Incident Management
Process
· Incident Management
A structured assessment used to identify hazards or uncertainties, estimate their risk and determine whether controls or further action are required.
Risk Assessment
Process
· Risk
A grid that combines likelihood and consequence or severity ratings to support consistent risk evaluation and prioritization.
Risk Matrix
Method
· Risk
An action taken to eliminate the cause of a detected nonconformity, incident or other unwanted condition and reduce the chance of recurrence.
Corrective Action
Process
· CAPA & Incidents
An action taken to eliminate the cause of a potential nonconformity or undesired situation so that it does not occur.
Preventive Action
Process
· CAPA & Incidents
