Definition
CAPA closure is the documented decision that a corrective or preventive action process can be formally closed. Closure should be based on evidence that required actions were implemented, relevant records are complete, unresolved issues have been addressed and effectiveness has been verified where the CAPA requires it. The term is a practical governance concept rather than one universally defined closure formula: closure criteria should be appropriate to the issue, risk and applicable requirements.
Why it matters
For Quality teams, completed tasks do not automatically mean a CAPA is ready to close. Premature closure can hide overdue follow-up, missing evidence or recurrence that becomes visible only later. Explicit closure criteria help reviewers distinguish implementation from effectiveness and create a defensible record of why the CAPA was considered complete.
Verify that agreed corrective or preventive actions were carried out as planned and that implementation evidence is available.
Confirm required actions were implemented
1
Ensure the problem statement, investigation, cause, decisions, actions, dates, responsibilities and supporting evidence form a coherent record.
Review the CAPA record for completeness
2
Do not hide overdue actions, rejected reviews, introduced risks or dependencies behind a closure status; address or formally transfer them.
Resolve or explicitly manage open issues
3
Use predefined criteria and an appropriate observation period to determine whether the action changed the intended condition in practice.
Verify effectiveness where required
4
Record who reviewed the evidence, the conclusion reached and why the CAPA is considered complete.
Document the closure decision
5
Keep relevant conclusions available for recurrence monitoring, management review, audits and future investigations.
Preserve learning and trend signals

Process explained
What should be confirmed before CAPA closure
Closure is a decision based on evidence, not a status reached simply because the final task was marked complete. The exact criteria should reflect the CAPA's scope and risk.
A structured process for addressing causes of actual or potential problems through corrective or preventive measures and verifying that actions are effective.

Preventive Action
Process
· CAPA & Incidents
An action taken to eliminate a detected nonconformity or immediate problem; it fixes what is wrong now but does not necessarily eliminate the cause.
Correction
Action
· CAPA & Incidents
A planned, evidence-based check that confirms an implemented corrective action or CAPA achieved its intended result and remains effective.

Method
· CAPA & Incidents
A systematic approach for identifying the underlying causes of a problem or event instead of addressing only its immediate symptoms.
