Definition

A Fishbone Diagram, also called an Ishikawa or cause-and-effect diagram, is a structured visual method for identifying and organizing possible causes of a defined problem or effect. Potential causes are grouped into categories and broken down into deeper contributing factors, giving cross-functional teams a common view of what should be investigated. The diagram generates and structures cause hypotheses; evidence is still needed to determine which causes actually contributed to the event.

Why it matters

For incident, nonconformance and CAPA investigations, the Fishbone Diagram helps teams avoid jumping too quickly to a single explanation. It brings together knowledge from operations, Quality, maintenance, engineering and other functions, making it easier to examine people, methods, equipment, materials, measurement and environmental factors before corrective actions are selected.
