Definition

A deviation is an unplanned departure from an approved procedure, process, specification, instruction or expected operating condition. In regulated and quality-controlled environments, deviations are normally documented, assessed for impact and investigated in proportion to their significance. A deviation may be isolated, may reveal a nonconformance, or may trigger root cause analysis, corrective action or CAPA when the event is significant or recurring.

Why it matters

For Quality and operational teams, deviations are an important early signal that actual work has moved away from the approved state. Managing them consistently helps distinguish minor one-off events from systemic issues, assess product or process impact, capture containment, connect investigations and CAPA, and identify recurring patterns before they become larger compliance or performance problems.

Describe the observed departure, affected product or process, timing and available evidence while the facts are still accessible.

Detect and record the deviation

1

Protect affected outputs or operations and evaluate quality, safety, regulatory or customer consequences.

Contain and assess impact

2

Use proportionate Root Cause Analysis when the deviation is significant, recurring or not adequately explained by the initial facts.

Investigate the cause where needed

3

Determine how affected material, records or processes will be handled and whether correction, Corrective Action or CAPA is required.

Decide disposition and required action

4

Confirm required actions and approvals are complete, retain evidence and record a justified closure decision.

Verify actions and close

5

Use repeated deviation types or causes to identify wider process weaknesses that may require systemic improvement.

Trend recurring deviations

Event explained

Managing a deviation from detection to disposition

A deviation records a departure from an approved process, requirement or expected condition. The response should establish what happened, control immediate impact and decide whether investigation, Corrective Action or broader CAPA is proportionate.
