Definition
Document acknowledgement is a recorded confirmation that an assigned user has reviewed a specified controlled document or document version and acknowledged its content. Organizations may describe this as read and acknowledge, read and understood or document familiarization. It is an operational control rather than a universally defined regulatory term, and acknowledgement alone should not be treated as proof of training, competence or effective task performance.
Why it matters
When procedures or instructions change, Quality and QHSE teams often need evidence that the affected people received the current version rather than relying on a broadcast email or an informal assumption. Linking acknowledgement to a named user, document version and completion date provides clearer follow-up and audit evidence, while keeping the distinction between simple awareness and demonstrated competence.
Identify the users, groups, roles or organizational units for whom the released or revised document is relevant.
Assigned audience
•
Connect the acknowledgement to the exact controlled version so historical evidence remains meaningful after later revisions.
Specific document and version
•
Use a target date and reminders where timely awareness is important, rather than leaving acknowledgement open-ended.
Expected completion date
•
Capture the user, date and completion status so the organization can demonstrate who acknowledged the document.
Recorded completion evidence
•
A read-and-understood confirmation can support awareness, but higher-risk work may still require training, assessment or formal authorization.
Separate acknowledgement from competence

What useful document acknowledgement evidence contains
Acknowledgement is most useful when it identifies exactly who was expected to review which version and when. It should support follow-up without being confused with competence assessment.
The process of ensuring that approved documents are current, traceable, accessible to the right people and protected against uncontrolled change or use.
